As a highly acclaimed Pediatrician and Reproductive Health specialist, as well as the Founder and Medical Director of CHILD Accra, Dr. Tuakli is deeply familiar with the current state of African healthcare and the opportunities for improvement. Dr. Tuakli also brings 10 years of experience on the Mercy Ships International Board. Read her insights below into what the theme for this year&#8217;s World Health Day, &#8220;Health for All,&#8221; really looks like on the continent of Africa.<\/p>\n\t<p>We celebrate World Health Day on April 7, 2023, and I thought it is a good time to reflect on what &#8220;Health for All&#8221; means on the African continent.<\/p>\n<p>En 2015 et 2017, l\u2019Assembl\u00e9e mondiale de la sant\u00e9 (AMS) a adopt\u00e9 la r\u00e9solution 68.15 et la d\u00e9cision 70(22). Ces r\u00e9solutions reconnaissent les soins chirurgicaux et anesth\u00e9siques comme essentiels \u00e0 la Couverture Sanitaire Universelle (CSU) et attendent du directeur g\u00e9n\u00e9ral qu\u2019il rende compte des progr\u00e8s de leur mise en \u0153uvre.\u00a0<a href=\"https:\/\/apps.who.int\/gb\/ebwha\/pdf_files\/WHA68\/A68_R15-en.pdf\" target=\"_blank\" rel=\"noopener\">68.15<\/a>, and\u00a0<a href=\"https:\/\/apps.who.int\/gb\/ebwha\/pdf_files\/WHA70-REC1\/A70_2017_REC1-en.pdf\" target=\"_blank\" rel=\"noopener\">decision 70(22)<\/a>\u00a0respectively-resolutions which recognised surgical and anaesthesia care as essential for universal health coverage and required the director-general to report on the progress of its implementation.<\/p>\n<p>Si des progr\u00e8s consid\u00e9rables ont \u00e9t\u00e9 r\u00e9alis\u00e9s au niveau international pour am\u00e9liorer la pratique des soins chirurgicaux et anesth\u00e9siques, les choses n\u2019ont malheureusement pas progress\u00e9 de la m\u00eame mani\u00e8re sur le continent africain. In order for &#8220;Health for All&#8221; to become a reality, access to safe, timely, and affordable surgery and ongoing surgical education must become a priority in Africa. The truth is very few people on the continent have received the life-saving surgery they need. An ASOS study showed that surgical operations conducted in 25 countries were\u00a0<a href=\"https:\/\/www.asos.org.za\/ASOS%20Lancet%20editorial.pdf\" target=\"_blank\" rel=\"noopener\">20 fois inf\u00e9rieur<\/a>\u00a0than the crucial surgical volume required to meet a country&#8217;s essential surgical needs each year (defined as 5,000 operations per 100,000 people). African surgical patients are also twice as likely to die after planned surgery than the global average, with 94.1% of those deaths happening in the 24 hours after surgery.<\/p>\n<p class=\"translation-block\">La couverture sanitaire universelle (CSU) signifie que tous les individus re\u00e7oivent les services de sant\u00e9 dont ils ont besoin sans cons\u00e9quences sur leur budget. Elle comprend l\u2019ensemble des services de sant\u00e9 essentiels et de qualit\u00e9, de la promotion de la sant\u00e9 \u00e0 la pr\u00e9vention, en passant par le traitement, la r\u00e9\u00e9ducation et les soins palliatifs.<\/p>\n<p>To make that happen, a number of things need to be in place. For starters, we need strong political leadership to prioritise the acceleration of access to surgical, obstetric, and anaesthetic care across Africa &#8211; not just because it&#8217;s the right thing to do, but because from a socio-economic standpoint, Africa can&#8217;t afford not to. En \u00e9duquant les individus et les communaut\u00e9s, ils seront en mesure d\u2019exiger l\u2019acc\u00e8s aux services de sant\u00e9 dont ils ont besoin, sans tomber dans les difficult\u00e9s financi\u00e8res, ou perdre la vie parce qu\u2019ils ne savaient pas que leur maladie pouvait \u00eatre trait\u00e9e chirurgicalement, et en connaissant les modalit\u00e9s de l\u2019acc\u00e8s \u00e0 ces soins chirurgicaux n\u00e9cessaires.<\/p>\n<p>One thing that does speak loudly to me right now is how we need to accelerate women&#8217;s access to obstetric and non-obstetric surgical care. When I&#8217;ve walked past the line of patients waiting to come on board Mercy Ships, I am always struck by how few women are coming there for their own care. The women I see are clearly in the role of carer, coming to support family members.<\/p>\n<p>We have seen improvement on the continent in relation to obstetric outcomes as more women are able to access safe Caesarean sections, but there is a need to focus on a wider array of conditions and procedures that could have life-changing impacts on women. We also need to ensure that when funds are allocated for care, we make a point of ensuring that women&#8217;s care is specifically included, rather than them getting what is left over.<\/p>\n<p class=\"translation-block\">Je ne dis pas qu\u2019il faut oublier les hommes. Je dis que des soins chirurgicaux s\u00fbrs en g\u00e9n\u00e9ral sont extr\u00eamement importants, et qu\u2019ils le sont aussi bien pour les hommes que pour les femmes. Tout le monde m\u00e9rite un niveau minimum de soins de sant\u00e9. It means every man, woman, and child, should be able to access high-quality surgical care and post-operative care, regardless of age, race, ethnicity, gender, income, or religion.<\/p>\n<p>Nous devons relever le niveau des attentes, r\u00eaver grand pour le continent, nous tenir mutuellement responsables et acc\u00e9l\u00e9rer l\u2019acc\u00e8s \u00e0 des soins chirurgicaux, obst\u00e9tricaux et anesth\u00e9siques de qualit\u00e9 dans toute l\u2019Afrique. C\u2019est possible car de nombreux gouvernements mettent concr\u00e8tement en \u0153uvre des plans strat\u00e9giques au lieu de se contenter d\u2019en parler, et des organisations incroyables comme Mercy Ships fournissent gratuitement une formation chirurgicale continue ainsi que des soins chirurgicaux s\u00fbrs aux patients des pays qu\u2019elles desservent.
